Investigate the Turning Leaf Life Span System of Care and practice integrated care coordination for patients/clients with complex needs. Explore the potential for developing a whole health, family-centered and multi-dimensional community-based system of care as indicated by the American Psychological Association, Health Care for the Whole Person Statement of Vision and Principles (2009).
